There is no cost or
obligation to open a Great Lakes Scrip Center account. We just need some basic
information about your non-profit organization and how you will be paying for
your orders. We will also need proof of your non-profit status to complete the
process.
Here’s what to do:
Step 1
Complete our on-line enrollment form.
Step 2
Print out a copy of the form and sign it.
Make a copy of your proof of non-profit status and a copy of a voided
organization (not personal) check, if you’d like to pay us via ACH electronic
payment (also the fastest way to get your orders).
Proof of
non-profit status can be:

A copy of the IRS notice
acknowledging you as a 501(c)3 organization
·
A letter of determination, from
your state, recognizing you as a non-profit organization
·
A statement, on your
organization letterhead, indicating the purpose of your fundraising activities
Step 3
Mail or fax all of the information requested above to Great Lakes Scrip Center, per the
instructions on the form (fax is the fastest). You should hear back from us
within 2 business days that your account is open.
